Photo Release Form

I hereby grant permission to Mountain Land Physical Therapy to take photographs or videos of me, and to use them for promotional or educational purposes. I also grant permission to edit, publish, and distribute such photographs or videos for the purposes mentioned above, without compensation or notification.

I understand that the photographs or videos may be used in print or digital media, including websites, social media, brochures, flyers, advertisements, and other materials. I also understand that the photographs or videos may be used in perpetuity, meaning that they may be used by the organization or photographer at any time in the future.

I release Mountain Land Physical Therapy from any and all liability, claims, or damages arising from the use of the photographs or videos, including without limitation any claims for libel or invasion of privacy.
